End-of-life planning is not a topic that we usually prefer to engage in - but it is one of the most important topics to consider for ourselves and those we love most. This workshop will review advance care planning, specific advance directives in New York, and introduce the Dementia Values & Priorities Tool - a free tool to help us document our wishes regarding the care we want - or do not want - if we are ever living with dementia.
