Eldersource Care Management

A care manager is a social worker trained in geriatrics. A care manager can assess your situation, discuss a care plan, link you to services or provide guidance about a move to a higher level of care.

Care management usually includes an in-home assessment which is a review of an elder’s housing, health, finance and safety situation as well as the ability to perform Activities of Daily Living (e.g. the ability to prepare a meal, handle finances, accomplish personal care, etc). We also provide a care plan with recommendations for support services and/or housing options and assist with plan implementation. Learn more here.

Specialized Services for Families Caring for a Loved One with Dementia

Our services specific to dementia caregiving include care coordination, healthcare coordination, family consultation, respite, workshops, and support groups. We provide services in Monroe and other Finger Lakes counties. Learn more here.

Family Mediation & Conflict Coaching

When a loved one is challenged by increasing frailty and/or chronic illness, family members share that journey. Sometimes in different ways. Healthcare Coordination

Respite

Respite provides caregivers with much needed breaks to take time for themselves or run errands. Lifespan’s Partners in Caring Respite program matches trained volunteers with caregivers of those with early stage dementia or noticeable forgetfulness. Call us at 585-244-8400 for more information.
