Celebrating identity, connection, and the stories that continue to shape us.

Through a professional, person-centered approach, our facilitator, Jacqueline Langberg, LMSW guides families weekly over five consecutive Tuesdays in creating a personalized legacy book for and with their care partner living with dementia. The book honors the life story identity, relationships, achievements, and lasting impact of our care partner while affirming that they remain valued and meaningful in the present- not simply remembered from the past.

Book-making materials will be provided!

Audience: Caregivers and their care partner living with dementia/memory loss.

This series is hosted by the Finger Lakes Caregiver Institute at Lifespan. In collaboration with: Touching Hearts at Home.
