Lifespan is certified by New York State to provide comprehensive service coordination for adults age 45 and older who have a documented (prior to age 22) disability. In our coordination role, our professional staff members provide guidance, information and links to supportive services including housing, chore service, transportation, personal care, financial management, counseling, crisis intervention, employment and respite.
